Hiccoughs

OVERVIEW

Hiccoughs or hiccups refer to frequent involuntary intermittent contraction of the diaphragm, often occurring several times a minute

CAUSES

  • diaphragmatic irritation: e.g. subphrenic abscess, cholecystitis, pneumonia, pericarditis
  • stomach wall irritation: e.g.distension, ulcer, ileus
  • phrenic nerve stimulation/irritation: neoplasm, goitre, epicardial pacing
  • brainstem lesion: neoplasm, ischaemia, surgery
  • metabolic: uraemia
  • psychogenic

May be difficult to distinguish from myoclonus and tics

MECHANISM

  • uncertain

MANAGEMENT

  • often unsatisfactory
  • diagnose and treat cause
  • medication: all have poor efficacy (e.g. chlorpromazine, haloperidol, phenytoin, carbamazepine)
  • physical stimulation of posterior pharynx by NGT may interrupt reflex arc
  • if persistent and fatiguing  phrenic nerve block has been tried

Chris is an Intensivist and ECMO specialist at The Alfred ICU, where he is Deputy Director (Education). He is a Clinical Adjunct Associate Professor at Monash University, the Lead for the  Clinician Educator Incubator programme, and a CICM First Part Examiner.

He is an internationally recognised Clinician Educator with a passion for helping clinicians learn and for improving the clinical performance of individuals and collectives. He was one of the founders of the FOAM movement (Free Open-Access Medical education) has been recognised for his contributions to education with awards from ANZICS, ANZAHPE, and ACEM.

His one great achievement is being the father of three amazing children.
