A 67 year old man is admitted with ?TIA. Routine CXR.

Study and report the CXR. What procedure has he had?

CHEST X-RAY INTERPRETATION

There is white out of the left hemithorax with pronounced mediastinal shift to the left and raised left hemidiaphragm. Right lung parenchyma is normal.


CLINICAL CORRELATION

This man had a pneumonectomy 2 years ago for lung cancer.


CLINICAL PEARLS

As a general rule, never venture into a post-pneumonectomy space with a needle or drain, unless you are the surgeon that made it.
