Volutrauma

OVERVIEW

Volutrauma = complication from mechanical ventilation that may manifest as

  1. extra-alveolar air and/or
  2. acute ventilator induced lung injury.

CLINICAL MANIFESTATIONS

EXTRA-ALVEOLAR AIR

  • caused by alveolar rupture and produce:
  • interstitial emphysema
  • mediastinal emphysema
  • pneumothorax
  • pneumoperitoneum
  • subcutaneous emphysema

Deterioration can be subtle or lead to overwhelming collapse

-> respiratory compromise
-> palpable subcutaneous emphysema
-> haemodynamic instability (tension pneumothorax)

ACUTE LUNG INJURY (FROM VENTILATION)

  • hard to detect (can analyse BAL fluid, but hard to differentiate from inflammatory process causing initial lung injury)

INVESTIGATIONS

  • these are aimed at identifying cause and assessing severity
  • CXR
  • ABG to quantify degree of respiratory compromise
  • CT (if indicated)

MANAGEMENT

  • lung protective ventilation (low TV, low MV, lower mean AWP, avoidance of auto-PEEP)
  • chest drains (drain extra-alveolar gas)
  • double lumen tubes and/or differential lung ventilation occasionally required

Chris is an Intensivist and ECMO specialist at The Alfred ICU, where he is Deputy Director (Education). He is a Clinical Adjunct Associate Professor at Monash University, the Lead for the  Clinician Educator Incubator programme, and a CICM First Part Examiner.

He is an internationally recognised Clinician Educator with a passion for helping clinicians learn and for improving the clinical performance of individuals and collectives. He was one of the founders of the FOAM movement (Free Open-Access Medical education) has been recognised for his contributions to education with awards from ANZICS, ANZAHPE, and ACEM.

His one great achievement is being the father of three amazing children.
