MECHANISM OF ACTION

  • inhibition of cell wall synthesis

PHARMACEUTICS

  • IV or IM
  • also known as penicillin G

DOSE

  • adults: 1.2-2.4g Q4-6 hrly
  • paediatrics: 30-50mg/kg Q6hrly

INDICATIONS

Infection:

  • Strep pyogenes
  • Gram positive bacilli (Clostridium)
  • Gram positive cocci
  • Gram negative cocci
  • Spirochaetes

Prophylaxis:

  • endocarditis
  • orthopaedics
  • surgery
  • post splenectomy

CONTRAINDICATIONS

  • hypersensitivity
  • resistant organism
  • high dose in renal dysfunction

ADVERSE EFFECTS

  • superinfection (pseudomembranous colitis)
  • GI upset
  • seizures
  • fever
  • interactions
  • physical incompatibilities

PHARMACOKINETICS

  • Absorption – bioavailability 30-60%
  • Distribution – 60% protein bound, Vd 0.75L/kg
  • Metabolism –
  • Elimination – 80% urine, t1/2 = 0.7 hr
