Intracranial calcification DDx
Overview
Intracranial calcification may be divided into neoplastic, vascular, infectious or miscellaneous causes; basal ganglia calcification suggests specific underlying causes.
Causes
Tumours
- glioma (astrocytoma, oligodendroglioma)
- meningioma
- craniopharyngioma (90% in children)
- ependymoma
- pinealoma
- teratoma
- chordoma
- dermoid
- cholesteatoma
- lipomas are usually midline and have typical curved bands of calcification
Vascular
- atheroma, aneurysm
- angioma
- subdural haematoma
- intracerebral haematoma
Infection and infestation (often multiple)
- Tuberculosis (basal meninges)
- toxoplasmosis (paraventricular)
- cytomegalovirus
- cysticercosis (usually supratentorial)
- pyogenic abscess
- hydatid
- Paragonimiasis
Miscellaneous
- Phakomatosis: tuberous sclerosis, Sturge–Weber syndrome (characterised by tram lines), von Hippel–Lindau syndrome, neurofibromatosis types 1 and 2, and Gorlin’s syndrome.
- Renal failure plus dialysis

- Birth anoxia (idiopathic (most common) bilateral and symmetrical)
- radiation
- mitochondrial diseases
- Fahr’s syndrome
- hypoparathyroidism
- pseudohypoparathyroidism

Chris is an Intensivist and ECMO specialist at The Alfred ICU, where he is Deputy Director (Education). He is a Clinical Adjunct Associate Professor at Monash University, the Lead for the Clinician Educator Incubator programme, and a CICM First Part Examiner.
He is an internationally recognised Clinician Educator with a passion for helping clinicians learn and for improving the clinical performance of individuals and collectives. He was one of the founders of the FOAM movement (Free Open-Access Medical education) has been recognised for his contributions to education with awards from ANZICS, ANZAHPE, and ACEM.
His one great achievement is being the father of three amazing children.
