Mediastinal widening on portable x-rays
Overview
The mediastinum may appear widened on a portable chest x-ray even for many reasons (other than the mediastinum actually being widened!)
Causes
- AP versus PA technique enlarges anterior structures because they are further from the imaging cassette
- Short distance between imaging cassette and x-ray source further magnifies anterior structures
- Supine versus upright positioning causes mediastinal soft tissues to flatten and become broader and the superior vena cava to become distended
- Shallow inspiration
- Rotated positioning of the patient
- Lordotic projection magnifies anterior structures

He was one of the founders of the FOAM movement (Free Open-Access Medical education) has been recognised for his contributions to education with awards from ANZICS, ANZAHPE, and ACEM.
